Action Planning Template Draft #1

Here is my Action Research Project Draft #1.  I know my goal is two general.  I'm looking to incorporate another word instead of effective.  I want to somehow use the quantitative data that I'm collect and analyzing which is students who earn D's and F's.  Any suggestions?
